If you have not enrolled in any courses yet at WSU - you should complete that process first through MyWSU, then attach a copy of your unofficial transcript showing current enrollment.

By submitting this application, I certify, understand, and agree to the following:

1. The information given in this application is complete and accurate, and I understand that to make false or fraudulent statements or withhold information requested within this application may result in disciplinary action, denial/revocation of admission and invalidation of credits or degrees earned.

2. I understand that all aspects of this application, including transcripts, become and remain property of Wichita State University and cannot be returned.

3. I understand this application does not require an application fee.

4. I understand that if I need to change my certificate program or add an additional program this will require an additional application.

5. I understand that completion of this application does not mean admission into a degree-bound program.

6. I understand the Graduate School may, at its discretion, provide programs additional information for review not contained in this submission.

7. I understand the certificate program coordinator may require additional information from me in order to review my application. That information will be submitted to the program directly and will not become a part of this application file.

8. I understand that if, for any reason, the Graduate School determines I am not currently in an eligible program or am not eligible for continued enrollment this application will not be processed and I will need to submit an application for admission.
